1. Purpose
This form satisfies the Platform's beneficial-ownership disclosure requirements consistent with FATF Recommendation 24, the EU 5th Anti-Money Laundering Directive, the US Corporate Transparency Act (where applicable), and FATF-equivalent regimes in the five operating jurisdictions.
2. Disclosure Threshold
Disclose all beneficial owners holding, directly or indirectly, 25% or more ownership or control over the Grantee organization. For each beneficial owner, provide:
- Full legal name as it appears on government identification.
- Date of birth.
- Country of citizenship and country of residence.
- Ownership percentage (direct + indirect).
- Politically Exposed Person (PEP) status.
- Sanctions screening attestation (will be re-screened by the Platform).
3. Indirect Ownership
For ownership held through entities or trusts, look-through to the ultimate natural persons. Disclose intermediate entities only if they themselves hold >25% directly.
4. Updates
You must update the disclosure within 30 days of any change in beneficial ownership exceeding 5 percentage points or any change in PEP status. Failure to update is grounds for Bad-Actor Registry inclusion.
